Start Services with Five Pillars
Community First Services and Supports (CFSS)
CFSS services are designed to support individuals who need assistance with daily living activities. Our goal is to help individuals maintain independence while receiving the care they need in a safe and respectful environment.
Personal Care Support Assistance with activities of daily living such as bathing, grooming, dressing, hygiene, and mobility support.
Household Support Help with meal preparation, light housekeeping, laundry, and maintaining a clean and safe home environment.
Community Support Support with attending appointments, running errands, and participating in community and social activities.
Companion Support Providing supervision, companionship, and encouragement to promote safety, independence, and overall well-being.
Private Pay
Private Pay Services
We understand that not everyone qualifies for waiver services or may need additional support beyond what is authorized.
Five Pillars Community Supports offers private pay options that provide flexible, personalized care without requiring case manager approval.
Private pay services allow for faster start times and customized support based on individual needs.
Who Can Receive Services
Individuals who qualify for CFSS or other Minnesota home and community-based services may be eligible for support through Five Pillars Community Supports.
We work with individuals receiving services through various waiver programs and support systems.

Disability Waiver Services (245D) – Coming Soon!
Five Pillars Community Supports plans to expand services to include Minnesota 245D Home and Community-Based Services (HCBS). These services will further support individuals in developing independence, life skills, and meaningful community engagement.
Services will be available upon licensure.
Individualized Home Supports (IHS) Support designed to help individuals live independently in their own homes while receiving personalized assistance based on their needs.
Independent Living Skills (ILS) Training and support focused on building essential life skills such as cooking, budgeting, cleaning, and managing daily routines.
Community Participation Support Support that helps individuals engage in community activities, build relationships, and participate in meaningful experiences outside the home.
Companion Services Non-medical supervision and companionship that promotes safety, socialization, and overall well-being.
Respite Care Short-term support services that provide relief for primary caregivers while ensuring individuals continue to receive quality care.
